Jackie may consider, for instance, that Sarah is incapable of giving her informed consent and that it is justifiable for her to gain the consent of a parent or carer. A further issue that complicates Jackie’s position with regard to this ethical dilemma is that her values may conflict with those of her profession and other people in Sarah’s life such as her parents or carers. Whereas Jackie may come to the conclusion that Sarah is an adult who knows her own mind, her manager may feel it is Jackie’s role and duty as a physiotherapist to mobilize Sarah’s knee.

Jackie would need to be sure, for example, that Sarah’s unwillingness to cooperate is not simply an expression of fear or a lack of understanding of the outcome. She also needs to be sure that Sarah is still interested in sport, she is not unduly depressed or anxious, and pain or distress is not distorting her judgement. It is a mistake to think that people are necessarily irrational because they are experiencing symptoms, but if pain is a problem for Sarah Jackie could give a pain-relieving treatment, such as heat or massage, before commencing Sarah’s exercises.
